Required for independent contractors to provide the information necessary for tax reporting purposes.
Direct Deposit Authorization Form
Complete this form to authorize electronic payments. Please double-check your banking information for accuracy to prevent payment delays.
Confidentiality & Non-Disclosure Agreement (NDA)
All team members are required to review and sign our Confidentiality & Non-Disclosure Agreement to protect sensitive company information, client records, and internal business processes.
Your NDA will be sent separately through TaxDome for electronic signature. You do not need to download or upload this document manually. Please review and sign it when you receive the request in your portal.
Document Submission
For the security of your personal and financial information, all completed forms must be uploaded through your secure TaxDome portal.
Please do not email completed forms containing sensitive personal or banking information.
